
VEHICLE COLLISION REPORT FORM ON THE WEB
Although drivers are no longer required to complete a collision report when a police officer comes to the scene of a collision, many drivers still need to complete their own reports in those instances when no police officer is present. Approximately 10 percent of collisions are reported on a Vehicle Collision Report Form that is completed by the driver.
Drivers will no longer
have to go to the nearest law enforcement agency to pick up a form or
complete one. They will be able to complete the form at home where all
their information is located. This form is now available on Washington
State Patrol’s website and can be downloaded by clicking on one of the two
options below:

The form can be partially completed on the computer with the diagram and signature added after the form is printed. The form is easy to see and can be enlarged to see detail, if necessary. A typed form is legible and will lend itself to greater accuracy for the information you provide. To be valid the form must be signed and a copy retained at home for future reference. The completed form is then mailed to Collision Records Section.
